Customer Notice

  1. Failure to use the eye protection provided to the customer by the tanning facility may result in permanent damage to the eyes.
  2. Overexposure to ultraviolet light causes burns.
  3. Repeated exposure may result in premature aging of the skin and skin cancer.
  4. Abnormal skin sensitivity or burning may be caused by reactions of ultraviolet light to certain:
    • Foods,
    • Cosmetics, or
    • Medications, including:
      • Tranquilizers,
      • Diuretics,
      • Antibiotics,
      • High blood pressure medicines, or
      • Birth control pills.
  5. Any person taking a prescription or over-the-counter drug should consult a physician before using a tanning device.
  6. Pregnant women should consult their physician(s) before using a tanning device.
  7. A person with skin that always burns easily and never tans should avoid a tanning device.
  8. A person with a family or past medical history of skin cancer should avoid a tanning device.
  9. Acknowledgements/Agreements:
    • I certify that I am 18 years of age or older.
    • My signature and date acknowledges that I have read, and understood, the above customer notice and the warning signs posted in the entry area and tanning room(s).
    • In addition, I agree to wear protective eyewear.
    • By signing this document, I hereby agree to comply with instructions on the use of UVA tanning systems or DHA sunless tanning systems.
    • I hereby acknowledge that I have read and understand the Texas Medical Board Advisory Statement.
    • I agree to wait at least 24 hours in between UV tanning sessions per Texas Health Code.
    • I agree to use Tan Etc. facilities and equipment at my own risk and release the owners, operators, and manufacturers from any damage or harm I might incur from use of the facilities or products.
  10. No oral representation shall be binding on Tan Etc Tanning Salon.
